The 2016 Texas Primary Care and Health Home Summit was the fourth annual statewide conference focused specifically on expanding access to medical home for individuals and families in Texas. This Summit offered stakeholders the opportunity to learn about medical home models and best practices, and to interact with program experts at various stages of implementation. The 2016 Summit included sessions on readiness for value-based contracting; building effective teams; population health strategies; improving chronic care management and successful business models for the health home.
