The 2015 Summit focused on emerging trends in primary care including practices without walls and direct primary care. The summit offered opportunities to learn about new models including sessions on value-based contracting and building a successful ACO.The summit featured a number of projects made possible under the Medicaid 1115 waiver program that brought innovation to underserved Texans. In addition, we offered sessions on the elements of health homes; integrating behavioral health into primary care; using data to improve care; best practices in care transitions and medication adherence and engaging patients and families in their care.
