This session will review the historical context of primary care in the US including the research of Dr. Starfield and others that helped establish the foundational role of primary care in the health of individuals and communities. The session will then focus on innovations in care models including the patient-centered medical home; Center for Medicare and Medicaid Innovation models (CPC, CPC+, others) and briefly review the evidence for their impact.
