Historically, behavioral health and physical health services have existed in separate silos which limits integration and overall effectiveness. Oral health services have similarly operated along parallel but non-intersecting tracks with primary care.

A panel discussion with 3 key stakeholders in primary care in Texas – a practicing physician; a health policy specialist and a primary care association executive whose goal is to explore key issues and solutions to advance access to care for all Texans while improving health outcomes, reducing disparities, and lowering costs.

This session will review the historical context of primary care in the US including the research of Dr. Starfield and others that helped establish the foundational role of primary care in the health of individuals and communities. The session will then focus on innovations in care models including the patient-centered medical home; Center for Medicare and Medicaid Innovation models (CPC, CPC+, others) and briefly review the evidence for their impact.
