To create sustainable change this session will reference the foundational elements of, “The 10 Building Blocks of High-Performing Primary Care” written by Thomas Bodenheimer, MD; et al. The foundational elements are: engaging leadership, data-driven improvement, empanelment and team based care. The engaging leadership discussion will include examples of how to engage primary care practice leadership through process improvement training and developing quality improvement teams to maintain a culture of continuous quality improvement. Data-driven improvement topics will include discussion around how to select the appropriate key performance indicators consisting of quality and utilization measures as well as how to integrate data into quality improvement team discussion. The empanelment discussion will include examples of how to assess risks and patterns within your patient population and finally the team based care discussion will include how to expand practice roles based on population health data to promote comprehensive care.
