This interactive session will provide participants specific ideas of how their practice can implement or refine care management including the importance of care plans (example care plan included), and the use of data and measurement. Additionally, participants will learn ways to improve collaboration and coordination (sample collaborative care agreements included) across the care team and health professionals outside of the primary care practice, incorporating patient and caregiver goals and values.
