UnitedHealthcare Community Plan of Texas has embarked upon learning collaboratives with Providers to leverage provider capacity in assessing and addressing social determinant of health (SDoH) solutions.  In the learning collaboratives education is provided on patient assessments, referral support and fulfillment documentation.  Additionally, the providers are given assistance with gaps in the process and information on these services performed by UnitedHealthcare Community Plan of Texas.  The collaborative also provides the opportunity to share data and best practices with provider partners to inform and collaborate on the ongoing design of SDoH models.  Through these collaborations it is our goal to positively impact quality outcomes and develop proof of concept to effect payment reform.
