Lessons Learned Creating SDOH Assessment and Referrals

Collection of social determinants of health (SDoH) data provides insight  into barriers that individuals face. Identifying barriers is important to  create appropriate referrals to overcome these obstacles. Collecting data  about referrals allows for tailored program development. Patient Care  Intervention Center (PCIC) has partnered with a local Federally Qualified  Health Center (FQHC) system in Harris County to help their team assess SDoH  barriers for their patients and create targeted referrals.  PCIC’s system consists of a values-based  case management platform, an imbedded SDoH assessment and a closed-loop  referral system that creates real time alerts for agencies and  individuals.  PCIC combines this data  with the data it collects from 650+ agencies to track the use of community  services and allow for targeted programming.   Metrics collected from the FQHC’s use of the platform, including  successes, barriers and agencies response time to a referral will be  discussed.
