The IPCP program is an inter-disciplinary team consists of nurse practitioners, nurses, behavioral health professionals, dietitians, community health workers, and pharmacists.  The team works collaboratively to help the patient with self-management goals.  The NPs, nurses, behavioral health professionals, dietitians, and pharmacy team work with patients in the clinical setting while the CHWs work with the patients in home visits or in clinic appointments.  The team meets bi-monthly where a selected provider can discuss specified patients in detail and the team can create a central goal for the patient’s self-management.   Providers refer patients with chronic diseases to participate in the program.  The patients that participate in the program have the opportunity to work with a CHW on social determinants of health issues, pharmacist on medication issues, dietitian to work on food related issues, and behavioral health professionals to cope with life better.
