Since the 1960s, community health centers (CHCs) provide access to care for millions of Americans, including some of the most marginalized individuals and communities. In April 2021, the Episcopal Health Foundation (EHF) launched the Clinics Pathway Approach (CPA), a five year program through which 11 CHCs in Texas are being funded to engage in enhanced multi-level work to build fundamental capacities for population health work and value-based payment system design. EHF contracted with JSI to conduct the evaluation of the CPA grantee work. JSI and EHF developed a population-based payment reform readiness assessment tool, utilizing components of a variety of existing evidence-based tools. Through the administration of this tool and qualitative site visits and key informant interviews, we are determining best practices to sustainably conduct population health and to engage in payment reform.
