This session will discuss key drivers of change required to advance population health management and value-based payment readiness in safety net primary care settings. The discussion is in context of Episcopal Health Foundation’s (EHF) Clinic Pathways Approach (CPA), a multi-year funding initiative to advance care transformation within community clinics in Southeast Texas. For the next five years, CPA is funding up to 11 clinic grantees within EHF’s 57 county service area to evolve their care model and capacity to succeed in value-based payment models. To facilitate this change, CPA utilizes a combination of long-term direct funding, peer learning, and practice coaching.
