This presentation will show how a community clinic's internal culture and mindset about health, partnerships with local community organizations, and collaborative action shifted to meet needs around food security, reliable and affordable transportation, neighborhood safety and access to care.    Presenters representing two different perspectives, the funder and clinical provider, will share their learning and experiences regarding the Community Centered Health Home model, which focuses on increasing access to services addressing social and environmental issues that influence health outcomes. This presentation will demonstrate how one clinic progressed to address community prevention in tandem with its community partners. Based on more than four years of experience implementing the model, presenters will share a comprehensive perspective of challenges, successes, and learnings.
