Children with Medical Complexity (CMC) have increased care utilization and care coordination needs. Primary caregivers of CMC need care coordinators that are accessible, knowledgeable about their child’s medical narrative, and empowered to make changes to improve the quality of life of their children. Currently, the model is for care coordination to be the function of Managed Care Organizations. In Austin Texas, we embarked on a novel approach, integrating care coordination within the medical home through an innovative capitated payment model. This model is a new partnership between Blue Cross Blue Shield Medicaid Star Kids and the Children’s Comprehensive Care Clinic, a medical home that serves children with medical complexity. During this session, we will establish the unique and substantial care coordination challenges facing CMC. We will describe our model and demonstrate how it overcomes barriers to coordinated care. We will illustrate our model’s successes and provide it’s lessons learned.
