Texas Primary Care Consortium 2022 Annual Summit Plenary: Advanced Primary Care: Mapping Progress and the Path Forward
Advanced Primary Care (APC) by design and intent shifts the focus of primary care toward quality. In a departure from volume based model of care delivery, APC ensures patients are provided with high-quality care and allows clinicians to spend more time and provide comprehensive care to their patients. Additionally, if a patient needs to be referred to a specialist, APC practices have the ability to maintain that relationship and ensure that the patient’s care is thoroughly coordinated throughout the treatment process.
However, over the last decade, while the shift and adoption towards APC has been encouraging, it is evidently clear that “the payment model” could singularly accelerate or impede this transformation. Through this session, key leaders and stakeholders in the Texas primary care landscape share their reflections and perspectives on the progress to date and offer a direction for the way forward.
